Southwest Louisiana schools have announced registration dates for the 2014-15 school year:

A.A. Nelson Elementary: New student registration 8 a.m.-1 p.m. July 28-Aug. 1 for kindergarten through fifth grade. Call 217-4790.

Maplewood Elementary and Middle: Pre-kindergarten through eighth grade registration 8:30-11:30 a.m. July 31-Aug. 5. Parents/guardians must provide official documentation to verify the primary places of residence. Such verification shall include two copies of the following: current utility statements to include an electric bill with parent/guardian’s name as the customer; a notarized statement confirming residency and two current utility statements, including electric bill of the person whom the family is currently living; lease agreement; certified copy of any judicially ordered tutorship custody or guardianship of any minor child not domiciled or in custody of their natural parent; property tax statement if parent or guardian; voter registration card of parent or guardian; copy of current drivers licenses of parent or guardian. Parent/guardian should also provide child’s birth certificate, Social Security card, current health record or immunization card and 2013-2014 school year report card and/or test scores. Pre-K students must register in their school zone and provide all items listed above for registration to be complete along with one of the following income guidelines for the family members earning income: 2013 tax return, 2013 W-2, foster care reimbursement, Office of Community Services, SSI, Department of Children and Family Services, FITAP, child support, December 2013 pay stubs, unemployment benefits. LA Pre-K registration is not guaranteed at registration.

Sam Houston High: Seniors can register 8:30-11:30 a.m. Aug. 1; juniors, 8:30-11:30 a.m. Aug. 4; sophomores, 8:30-11:30 a.m. Aug. 5; and freshmen, 8:30-11:30 a.m. Aug. 6. Activity fees are $30 for all students, except those in the STEPS program. Senior fee is $30; parking fee is $15 for seniors and $10 for under-classmen (optional); yearbook fee is $45 (optional). All fees can be paid with one check. Call 217-4480.

Hathaway High: New student registration 8 a.m.-noon and 1-3 p.m. Wednesday, July 30. Parents should bring their child’s certified birth certificate, Social Security card, immunization records, proof of residency, and custody papers, if applicable. Frasch Elementary: New student registration 8-11 a.m. Aug. 4-8 in main office. Parents asked to bring child’s original birth certificate for verification (no copies), Social Security card and current immunization records, along with proof of residence including two utility bills, one of which must be an electric bill. Only in-zone students will be registered at this time.

R.W. Vincent Elementary: 9 a.m.-1 p.m. July 28-Aug. 1 in the front office. Parents/guardians of students in kindergarten through fifth grade must have the following documents for registration or an approved out-of-zone request: parent/guardian’s ID; two proofs of residence, one must be an electric bill the other a utility payment; ;otarized affidavit if the bills are not in the parents name; certified birth certificate; Social Security card; and, if applicable, copy of judicial order of custody if parents are divorced showing the domiciliary parent — must be signed by a judge — or copy of foster care placement or adoption.

Calcasieu Parish Alternative School: 9-10 a.m. and 1-2 p.m. Aug. 4-5.

Dolby Elementary: 8-11 a.m. July 31-Aug. 1. Documents required to register are a permit from the Office of Child Welfare and Attendance, child’s birth certificate, social security card and shot record. Assessment fees are $15 for kindergarten-fifth grade; $10 for pre-k students. Families can sign up to join PTO, register for Extended Daycare, and order yearbooks, spirit shirts and chair backs.
